Pilot and Controller Questionnaires

Completed pilot and controller questionnaires are analyzed, and the results are used to recommend and implement changes to the TCAS logic and displays. Because questionnaire data will be used to assess the operational performance, the continued support of both pilots and controllers is essential.

As soon as possible after a TCAS event, please complete the following steps:

1. Print and fill out the appropriate questionnaire.

   • Pilot Event Questionnaire, Version 7.0 (PDF, 36 KB)
   • Air Traffic Controller Event (PDF, 10 KB)
     
     

2. Mail the completed questionnaire to:

Information contained in the questionnaire will be used only by TCAS Transition Program participants to assess the operation of TCAS. Once the information is entered into the TCAS Transition Program database, the questionnaire will be destroyed.
